Fax us at (800-345-3748) or e-mail us at KenCoastal@gmail.com with all of the information we need to put on your prescription form. (A copy of your current Rx might suffice). If there are changes you wish to make, such as additional doctors or addresses, phone number or address changes, or format changes, be sure to detail those. We need doctors' names, degrees, state licenses, and DEA numbers. We also need addresses and phone numbers. Optional item are clinic name, doctors' specialties, FAX numbers, and similar items. Tell us how you want us to bill you or call us at 800-426-2782 for more information. For multiple doctors scripts we need complete information for each doctor. Make sure to give us your e-mail address or Fax number so we can send you a proof before printing.
